Yin and Yang: False heat and false cold
What it looks like is not always what it is. 6 activities, about 11 minutes.
When the extreme puts on a disguise
A large internal cold pushes what little yang is left to the surface: the patient looks hot and is in fact freezing. And the reverse: a large internal heat blocks circulation and leaves hands and feet cold.
- True cold with apparent heat
- red face but wants to wrap up
- True heat with apparent cold
- cold hands but throws off the blanket
- What decides
- what the patient prefers, not what you see
Common mistake: Diagnosing from the loudest sign. In false pictures, the loudest sign is precisely the one that lies.
Very red face, agitation and thirst… but they want hot drinks, use two blankets and the urine is clear. What is it?
Cold hands and feet, but high fever, strong breath, constipation and a rapid forceful pulse. What is it?
Facing a picture you suspect is false, which signs do you trust and which not?
- What drink temperature they prefer
- The colour of the face
- What the urine is like
- Whether the hands are cold
- Whether the pulse has force under pressure
- How much the patient complains
Order the questions that separate a false picture from a true one, from most decisive to merely confirmatory.
- Do they prefer cold or warmth, in drinks and in the room?
- What is the urine like, clear or concentrated?
- Does the pulse keep its force under pressure?
- Is there a coating, and what colour?
- What does the face look like?
In true cold with a very marked apparent heat, how do you warm without provoking a rejection?
