Pattern: Cold-Damp Bi in the knee. Supporting signs: Local heaviness and stiffness, Worse with rain and cold, Local swelling, Cold to touch, Deep tight pulse.

The deciding clue: No general coldness: No lumbar cold, fatigue or nocturia..

Safety: Recent flight plus a unilateral warm calf requires urgent assessment for thrombosis.

Treatment: Dispel Cold-Damp, Open channels; Warm and mobilise, Gentle local and distal. Points: ST35, SP9, GB34, ST36.
